The timed concentrations are obtained with routine blood draws (as established by Phlebotomy Services) during the dosing interval between the time of the peak and the trough. The requested time of collection must be included with the order for placement of the request on the appropriate phlebotomy draw schedule. Please refer to the Aminoglycoside Therapy Monitoring Policy Memo #97 or consult with the pharmacist for recommended monitoring guidelines.
